What information must the EVV system collect?

Any approved MDDHS EVV system must verify:

• The type of service performed
• Name of the patient who received the service
• The exact date of the service
• The location where the service was performed
• Name of caregiver who provided the service
• The exact start and end time of the service

Providers are free to use either the state-provided EVV system HHAeXchange, or their own Michigan EVV compatible software system
